[RESOLVED] Apex Adapter has Broke

My Adapter USB has broken off of the Adapter. This has happened before and I would like an email on what I need to do to get it to you, and where it must be shipped. Thank You for your time.

Broken Adapter

My Adapter USB ha broken off of the rest of the Adapter. I have had this problem before and you all have fixed it for me and I thank you for that. I would be grateful for you to fix it again. I am willing to pay the price of fixing. Please respond with information on how it may be fixed. Thank You.
